How to Get Results of a DOT SAP Follow-Up Testing Plan

It’s important to know how to get the necessary SAP reports and follow-up testing plan to ensure compliance. There are two main steps.

Step 1: THE EMPLOYEE/DRIVER Needs to Fill Out the Release of Information Form

This form gives written permission to release your private health records. It’s a crucial step because it authorizes the sharing of sensitive information with the necessary parties.

Step 2: THE EMPLOYER Requests Reports and Follow-Up Testing Plans

Once the Release of Information form is completed, the next step involves the employer. The designated employee representative or supervisor at the company must send an email to resilientreturn@gmail.com. This email should request the employee's SAP reports and follow-up testing plans.

Information to Include in the Email:

  1. Full company name
  2. Company address
  3. Company DOT number
  4. Supervisor’s full name
  5. Supervisor’s email address
  6. Supervisor’s phone number
  7. Last 4 digits of the employee's Social Security Number (SSN)

PLEASE NOTE: DOT regulations do not allow you as the employee/driver to have access to your follow-up testing plan. It has to be sent directly to the employer/CTPA that is managing and implementing it.
